Dental Insurance in California

Understand your options for preventive care and dental treatment.

Compare dentist networks, premiums, benefit limits, and the amount you could pay before choosing coverage.

Benefits, networks, waiting periods, and costs vary by product. A covered service may still leave you with out-of-pocket expenses.

What can dental insurance help cover?

Dental plans organize services into categories. The categories and payment amounts differ by product.

Diagnostic and preventive

Exams, cleanings, and X-rays

Basic treatment

Fillings and certain extractions

Major treatment

Crowns, bridges, and dentures

Other services

Root canals, gum treatment, implants, or orthodontics, depending on the plan

A service can fall into different categories under different plans. Some services may be excluded.

Does "covered" mean fully paid?

No. Payment can depend on the deductible, copayment or coinsurance, network, benefit limits, and treatment requirements.

Are all cleanings treated the same?

No. A routine cleaning and periodontal treatment can have different benefits and patient costs.

Ask the dentist for the specific services being recommended, then check them against the plan.

How do dental plans differ?

Dentist access

Dental PPO

Typically offers a network; some out-of-network benefits may be available

Dental HMO (DHMO)

Generally uses participating dentists and may require a selected primary dentist

Payment arrangement

Dental PPO

Often shares costs through deductibles and coinsurance

Dental HMO (DHMO)

Often uses a schedule of patient copayments

Annual benefit limit

Dental PPO

Common, but product terms vary

Dental HMO (DHMO)

Many products do not have an annual dollar maximum

Specialists

Dental PPO

Check network and referral requirements

Dental HMO (DHMO)

Referrals or authorization may be required

Main comparison

Dental PPO

Allowed charges, benefit percentages, and limits

Dental HMO (DHMO)

Participating offices, service availability, and copayment schedule

The product's documents control. Do not assume every PPO or DHMO works identically.

What is a dental discount program?

A discount program provides access to specified prices through participating providers. It is not insurance paying covered claims.

Confirm the provider, discounted fees, membership cost, and cancellation rules.

Carrier materials illustrate the differences between PPO and DHMO arrangements. California distinguishes discount programs from insurance. Delta Dental

What will I pay for dental care?

Your costs may include:

  • The premium to maintain coverage
  • A deductible before certain benefits apply
  • Copayments or coinsurance
  • Charges for excluded services
  • Amounts exceeding benefit limits
  • Additional out-of-network charges

What is an annual maximum?

An annual maximum is the most the plan pays toward applicable services during its benefit year.

It limits the plan's payment-not necessarily your spending.

After that amount is used, you may be responsible for further treatment costs. Some services may not count toward the maximum, depending on the product.

A simple example

Illustrative example only

Suppose a plan has a $1,500 annual maximum and has already paid $1,000 toward services that count against it.

There would be $500 remaining for applicable services that year. That does not mean the plan will pay the next $500 automatically; other coverage requirements still apply.

When does the maximum reset?

Check the benefit-year dates. Do not assume every product resets on January 1.

How should I compare total costs?

Consider the annual premium plus the amount you expect to pay for your planned care. Compare equivalent treatment and provider arrangements.

Carrier guidance defines annual maximums and explains that eligible services can count differently toward them. Delta Dental

Planning treatment? Check these details before enrolling

An active plan does not necessarily pay for every procedure immediately.

Is there a waiting period?

Some individual dental products require a period of enrollment before specified benefits become available. Others do not.

Check the actual product and service. If prior coverage can satisfy a requirement, confirm the conditions and required documentation.

What other limits should I check?

Is the proposed procedure covered?

A general category description may not answer this

Is prior authorization required?

Approval may be needed before treatment

How often can the service be repeated?

Frequency and replacement rules can affect payment

Is treatment already underway?

The product may have rules about when treatment began

Is a less costly treatment used to calculate benefits?

You may owe the difference for the option selected

Are implants and related procedures covered separately?

Each component can have different terms

Is there a separate service limit?

The general annual maximum may not be the only limit

What if I already have a dental condition?

Do not assume an existing condition automatically disqualifies the treatment.

California restricts preexisting-condition provisions in regulated dental coverage and prohibits waiting periods in large-group dental coverage. The rules are different from assuming every individual product has immediate benefits.

Ask for an explanation based on the applicable product and California requirements.

Does Medicare cover dental care?

Review existing dental benefits before paying for another product. Two plans do not automatically mean every bill is paid in full.

Original Medicare

Original Medicare generally does not cover routine dental care such as cleanings, fillings, extractions, dentures, or implants.

Certain dental services may qualify when closely connected to covered medical treatment. These are specific exceptions, not general dental coverage.

Medicare

Medicare Advantage

Some Medicare Advantage plans include dental benefits. Review the covered services, dentist network, patient costs, and benefit limits before adding separate coverage.

Other coverage

Check any employer, retirement, or other dental benefits you already have.

Qualifying Medi-Cal members may have dental benefits. Review current eligibility and covered services through the official program. CA.gov

Can I keep my dentist?

Verify participation in the exact product and network.

"Accepting insurance" can mean the office will submit claims. It does not necessarily mean the dentist is in network.

Confirm:

  • The dentist's name and office location
  • Participation in the specific network
  • Whether the office accepts new patients
  • Specialist participation and referral requirements
  • Expected out-of-network charges, if applicable

Ask for a written treatment plan

For significant work, ask the dentist to identify the recommended procedures and estimated charges.

Then request a pre-treatment estimate from the insurer or plan, where available.

Is the estimate a payment guarantee?

No. Final payment can change based on eligibility, services performed, remaining benefits, and other policy conditions.

Review the estimate before treatment and clarify anything you do not understand.

Carrier guidance expressly states that pre-treatment estimates are not guarantees of final payment. Delta Dental

What should I confirm before enrolling?

Check the effective date, waiting periods, benefit schedule, premium, and cancellation terms.

Before speaking with an agent, gather:

  • Your current dental coverage information
  • Your preferred dentist and office location
  • Any treatment estimate you already have
  • Your priorities, such as preventive care or dentures
  • The monthly amount you can comfortably afford

An agent can explain available products and their coverage terms. Your dentist determines the treatment appropriate for you.
